The Importance of Gum Health Before Dental Implants: Why Gums Come First?

For long-lasting dental implant treatment, healthy gums are essential. Why is gum treatment before implants so important? Discover pre-implant gum evaluation with Periodontology Specialist Dr. Özge Uysal at Papirus Dental Fethiye.

Why Is Gum Health So Important for Dental Implants?

Dental implants are one of the most modern and comfortable solutions for missing teeth. However, for an implant to function problem-free for many years, a good brand or a technically correct surgery alone is not enough. The real foundation of this treatment is healthy gums and healthy bone tissue.

At Papirus Dental Fethiye, especially in patients who are planning to have dental implants, we make it a principle to evaluate and treat gum health. Together with our Gum Disease and Periodontal Surgery Specialist Dr. Özge Uysal, many implant treatments are planned on a stable and healthy gum foundation.

In this article, we will answer the question “Why should gums come first?” in a detailed yet easy-to-understand way.

What Makes Gum Health So Critical for Implants?

A dental implant is a screw-like structure made of titanium–nickel alloy, optionally with zirconia-based components, placed into the jawbone. The tissues that surround this structure and form a barrier between it and the oral environment are the gums and the surrounding bone. If these tissues are not healthy:

  • The osseointegration process (the bonding of the implant to the bone) is at risk,
  • The likelihood of inflammation developing around the implant increases,
  • In the medium to long term, a serious condition called peri-implantitis – destruction of the bone and gum tissue around the implant – may occur.

For this reason, our professional approach in the clinic is not “Let’s place an implant for the missing tooth and think about the gums later”, but rather:

“Let’s first restore gum and bone health and then place the implant.”

Is a History of Periodontitis a Risk for Implants?

Research shows that in patients with a previous history of periodontitis (advanced gum disease), problems around implants and the risk of implant loss can be significantly higher compared with individuals who have healthy gums.

A patient who has previously experienced severe gum disease and is not treated properly or followed up with regular maintenance may face a significantly increased risk of developing peri-implantitis (inflammation and bone loss around the implant) over time. Some studies show that peri-implant diseases can occur two to three times more frequently in individuals with a history of periodontitis.

Therefore, in this group of patients, having a periodontist evaluate the case before deciding on implant treatment provides a major advantage.

What Do We Do During Pre-Implant Gum Evaluation?

At Papirus Dental Fethiye, every patient who applies for dental implant treatment, first undergoes a detailed assessment of gum and bone health. This evaluation is performed by our Periodontology Specialist Dr. Özge Uysal and generally includes the following steps:

1. Clinical examination

  • Assessing the colour, shape and overall condition of the gums, as well as any bleeding
  • Measuring periodontal pocket depths around each tooth
  • Evaluating signs such as tooth mobility and gum recession

2. Radiographic assessment

  • Using our Volumetric Tomography (3D CBCT) machine to evaluate jawbone height and density in three dimensions
  • Detecting any existing bone loss around the teeth and in potential implant sites

3. Analysis of risk factors

  • Smoking habits
  • Systemic diseases such as diabetes
  • Previous history of periodontitis
  • Daily oral hygiene routines and brushing / flossing habits

Based on this analysis, we provide a personalised, evidence-based answer to the question:

“Should we first treat the gums, or can we proceed directly with implants?”

In Which Cases Is Gum Treatment Essential Before Implants?

In the following situations, gum treatment is mandatory before planning any implant surgery:

  • Noticeable gum recession
  • Teeth that are loose or drifting apart
  • Advanced bone loss detected on volumetric tomography
  • Deep periodontal pockets next to the area where the implant is planned
  • Patients with a previous diagnosis of periodontitis that is currently uncontrolled

In such cases, we first stop the active inflammation in the gums. When needed, we perform detailed tartar removal, root surface debridement and surgical periodontal treatments. Only then do we create a more predictable and safer foundation for implant placement.

What Does Pre-Implant Gum Treatment Include?

Depending on the patient’s condition, pre-implant periodontal treatment may involve:

  • Professional tartar removal (scaling / debridement)
  • Root surface debridement (root planing / curettage) in deep pockets under local anaesthesia
  • Local or systemic antibiotic support when indicated
  • Surgical periodontal procedures in advanced cases (pocket reduction, bone reshaping, bone grafting)

The goal is to place the implant in the calmest and healthiest gum environment possible, so that we can offer the most reliable and long-lasting treatment outcome.

Why Are Regular Check-Ups and Maintenance Essential?

Protecting gum health after implants are placed is at least as important as the preparation beforehand. Studies in this area show that patients who attend regular maintenance visits and professional check-ups have a significantly lower risk of developing peri-implantitis.

For patients who receive implants at Papirus Dental Fethiye, we invite them to regular follow-up appointments, monitor the implants and surrounding bone radiographically, perform professional cleaning around implants when necessary, re-evaluate the quality of home care at every visit. In this way, any potential complications can be detected before they reach an advanced stage.
